The form asks for a few basics so we can identify your project:

• Phone

Permission Granted

By signing, you grant Crafted Memories Photo Albums LLC permission to use the following content created or supplied by you:

• Select photos submitted for album creation
• Final album spreads and design layouts
• Before/after restoration examples (if applicable)

These materials may be used in:

• Website galleries (craftedmemories.us)
• Social media platforms
• Printed samples & marketing books
• Online advertisements
• Promotional materials

Restrictions

You're in control of how far this permission goes. On the form, you can request any of the following:

Revoking Permission

The customer may revoke this permission at any time by submitting a written request to admin@craftedmemories.us.

Consent & Signature

By signing the form, you confirm that:

• You are the legal owner of the photos OR you have full rights to grant this permission.
• You understand how your images may be used.
• You grant Crafted Memories Photo Albums LLC permission as described above.

The form includes signature and date lines for both the customer and a Crafted Memories representative.
